Osteomalacia is a pathologic clinical feature caused by the impaired bone and cartilage mineralization. Clinical manifestations are not apparent. Vitamin D provides adequate calcium and phosphorus concentration for mineralization in extracellular matrix. In vitamin D deficiency mineralization is impaired. Clinical symptoms and signs of osteomalacia are mild and thus not useful for diagnosis. In severe cases bone pain and tenderness, muscle atrophy can be observed. If the diagnosis of the osteomalacia is delayed, it can easily be mixed up with osteoporosis which is observed in elderly people. The case which had been presented with diffuse muscle spasm, osteoporosis and pseudofracture reminded us about the late complications of gluten enteropathy and about how the diagnosis is delayed, and thus we decided to report it.
